Student Pronoun/Name Update Form

At SQA we would like students to be noticed for their gender pronouns and be able to feel comfortable expressing themselves. Please fill out this form if you would like to update us on your pronouns/name and allow us to update it on Infinite Campus with your permission. Only the school counselor Mrs. Franco will see the survey results, and only your teachers will see your pronouns/name.

*If you wish to change your gender pronoun/name in the future you can do so by resubmitting.
